1. Contact Your Insurer
Call the member services number printed on the back of your insurance card. Inquire if Addison Medical or our individual providers are listed as in-network for your specific policy.
2. Understand Co-pays & Deductibles
Co-payments, co-insurance, and deductibles are determined by your policy details. All co-pays are due at the time of your check-in appointment.
3. Referrals and Pre-Authorizations
Some HMO or managed care plans require a primary care physician referral before specialist visits. Please coordinate referrals with our administrative staff prior to scheduled consultations.
